If you would like to apply for membership to the Musicians' Hearing Health Scheme, you just need to complete a simple form.  We will ask you for your contact details and some evidence to show that you are a professional musician. Once you have sent us your application, we will make an assessment and Musicians' Hearing Services will be in touch within five working days to let you know if you are eligible.  You will then be registered onto the scheme and contacted to make an appointment at a hearing clinic of your choice.
